Through United Community network’s Unite Us shared technology platform, Norton Healthcare team members can exchange referrals with community resource partners who may help address patients’ health and social needs.

If you need help with essential services, please speak to one of your care providers about getting connected through the United Community network. The process is secure and confidential within the Unite Us platform.
When we refer you to a United Community partner agency, the agency should reach out to you in three to five days about its services. You may receive the call from a number you don’t recognize, so please be sure to have voicemail activated or answer all calls.
Our team that is monitoring referrals will contact the agency to follow up if no action has been taken.
If you are a Norton Healthcare patient and the community resource agency doesn’t respond to the referral or declines the referral, a Norton Healthcare team member will contact you for alternative options. Speak to your Norton Healthcare provider for more information.
