Retrieve your Underlying Medical Conditions

Norton Healthcare has opened COVID-19 vaccination registration for members of the Tier 1c group who are essential workers and patients 18 and older with chronic conditions, as defined by the CDC, that put them at highest risk for complications from COVID-19.

If your vaccination site requires verification of your underlying condition, Norton Healthcare patients can retrieve a list either through Norton MyChart or by filling out the form below to request your records. This information may be helpful as the state continues to rollout COVID-19 vaccinations to additional tiers.

How to Retrieve Your Underlying Medical Conditions From Norton MyChart

From your desktop computer:

Sign in to Norton MyChart
From the menu icon at the top left, scroll down to “Health Summary” under “My Record.”From the menu icon at the top left, scroll down to “Health Summary” under “My Record.”
Choose the “Current Health Issues” tab.
Print the record.

From your mobile device:

Download the MyChart app from either the Apple App Storeor Google Play and sign in.
Select the “Menu” icon from the center of your screen and scroll down to “Health Summary” under “My Record.”
At the bottom right, select “Health Issues.”

Show the Health Issues screen to verify your underlying health condition(s).
