Norton Healthcare wants to honor Jefferson County, Kentucky, teachers who are supporting our community’s students by sharing the importance of compassion, kindness and understanding. Selected teachers will receive school supplies for their classroom and special recognition during the third annual Unity Jam on Saturday, Aug. 8. Even better, the students who nominate the winning teachers will win a special backpack filled with school supplies.
All Jefferson County students are encouraged to nominate their teacher. Two teachers will be selected from each of the following grade levels: elementary, middle and high school.
Students should work with a parent or guardian to complete this form.
