Norton Healthcare (NHC) is pleased to offer you access to information about your visit as well as the services and resources offered at Norton Healthcare through our mobile applications.

We think it is important for you to know how we handle your information communicated via the Internet. These Terms & Conditions of Use outline NHC’s practices regarding Norton mobile applications information access and our sensitivity to the right to privacy. By using the Norton mobile applications, you confirm that you have read this document and agree to be bound by the terms, conditions and statements contained in this document. Norton Healthcare offers its MyNortonChart users health tracking tools for the benefit of patient understanding and reference. By connecting your health data to this application, you give Norton Healthcare providers access to view information logged. Unless you are under the direct care of a Norton Healthcare employed health care professional, this functionality is meant for informational and educational purposes only. Information gathered is not monitored by a health care professional, unless part of an established treatment plan. Therefore, if you have any questions or concerns about your health, you should contact your NHC provider directly.

Urgent Matters

You should never use Norton mobile applications for urgent matters. For all urgent matters, please contact a physician’s office by telephone, go to an emergency room or dial 9-1-1.

Security and Confidentiality

NHC is committed to protect the confidentiality of medical information. We afford the same degree of confidentiality to medical information received and transmitted by Norton mobile applications as is given to medical information stored in medical charts at NHC and other medium. We limit NHC employees’ access and ability to enter or view information based on their role. Firewalls, passwords, encryption and audit trails are further used to safeguard medical information. We have taken steps to make all information received from our online visitors as secure as possible against unauthorized access and use. You agree that if you accidentally gain access to another patient’s information, you will stop viewing it and will notify NHC by calling 888-4-U-Norton. In no event will you deliberately attempt to access information for any person other than your own information.

You understand that no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When you use Norton mobile applications, information will be transmitted over a medium that is beyond NHC’s control. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION, OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF THIS SERVICE. Once the information is received by us, medical information will be treated as confidential and given the same protection that all other NHC medical records are given.

Your Responsibilities

You agree that Norton mobile applications are a communication service offered as a convenience to you. You understand that it provides you with access to information about your visits and health maintenance, and that you are solely responsible for any sharing of such content that you intentionally or unintentionally communicate to others. You understand that messages will be routed to the appropriate NHC department as necessary for handling, and therefore NHC employees, other than your healthcare provider may be involved in addressing your request. Therefore use of the Nutrition Messaging may not be appropriate if there is sensitive information that you would only wish to discuss directly with a particular medical provider.

You understand that you may not transmit any information or message that contains unlawful, threatening, fraudulent, libelous, defamatory, obscene or abusive information or language. Norton mobile applications may not be used for any illegal or unethical activities. Hacking, attempted access or other forms of inappropriate access or attempted access to unauthorized areas or information on this site will lead to potential legal prosecution.

No Warranties

This application and the content contained herein are provided by NHC on an “As Is” basis. NHC makes no representations or warranties of any kind express or implied as to the operation of its sites, or the content, products and/or services included therein. To the fullest extent permissible by applicable law, NHC disclaims all warranties, express, or implied, including but not limited to implied warranties of merchantability, fitness for a particular purpose, title, and/or infringement.

Limitation of Liability

Some of the material on the NHC site is provided by third parties and NHC shall not be held responsible for any such third-party material. NHC disclaims any responsibility for, or liability related to, such third-party material. Any questions complaints or claims related to any product should be directed to the appropriate third party.


By using NHC’s web site and mobile applications, you agree that the laws of the Commonwealth of Kentucky, without regard to principles of conflict of laws, will govern these Terms and Conditions of Use and any dispute that might arise between you and NHC, its doctors, nurses, staff or other affiliates. You expressly agree that exclusive jurisdiction for any dispute with NHC, its affiliates, employees, subsidiaries, contractors, officers and directors, resides in the courts of Jefferson County, Kentucky, and you further agree and expressly consent to the exercise of personal jurisdiction in the courts of Jefferson County, Kentucky in connection with any claim involving NHC, its affiliates, employees, subsidiaries, contractors, officers and directors. Use of NHC’s web site is unauthorized in any jurisdiction that does not give effect to the terms and conditions set forth herein. NHC may modify this Terms and Conditions of Use at any time.
