Millions of people wear consumer devices that track everything from sleep to stress levels, heart rate to step count, and many things in between. Some wearable trackers claim to monitor your blood pressure. While that data may feel empowering, what do those numbers mean for your heart health and how accurate are they?
Think of wearables as a starting point for tracking your health data. If you have a chronic condition such as high blood pressure, diabetes or a family history of heart disease, a wearable health tracker may give you valuable trends to share with your healthcare professional. Remember, a health tracking wearable should not take the place of a blood pressure check and an annual physical exam with a trained healthcare professional.
What your smart ring or health tracking wearable can measure includes data on activity and fitness, heart health, sleep and some wellness. Some of this information is worth watching to get a baseline for what is “normal” for you. Consistently wearing your device shows your specific trends. A medical professional can look at those trends and determine if there’s an issue that needs further investigation. Many factors influence changes in your health, including diet, activity, stress and sleep. Looking at just one day of data from a smart ring isn’t enough. The trend over time is what is important.
Smart rings such as the Oura ring or the Galaxy ring, and wrist-worn fitness trackers such as those made by Fitbit or Garmin, advertise blood pressure tracking or alerts as ‘wellness features.’ They are not generally approved by the Food and Drug Administration for blood pressure measurement. At this time, wearable trackers don’t take the place of direct blood pressure measurement. Internet searches for “blood pressure ring” are rising fast. It makes sense — there is a rising interest in health and wellness, but in this busy world, people are looking for ways to save time and money. “These devices are interesting, and the technology is advancing rapidly. They may be useful for monitoring changes in trends and tracking lifestyle changes, but it does not take the place of a cuff measurement at this point,” said Kent E. Morris, M.D., MBA, electrophysiologist with Norton Heart & Vascular Institute.
The American Heart Association and the FDA warn against relying on unapproved cuffless blood pressure devices for clinical decisions, and with very good reason. High blood pressure, or hypertension, is one of the most important risk factors for cardiovascular disease and is underdiagnosed in the U.S. High blood pressure often has no symptoms, and wearable estimates can give you false reassurance. A clinically administered blood pressure test involves a cuff placed on the upper arm and inflated. Either the healthcare provider or a machine records the sounds of blood moving through the artery.
Blood pressure can be taken by a healthcare provider in two ways: auscultatory or oscillometric. Auscultatory means the provider fits you with the cuff, inflates it, and listens to your brachial artery with a stethoscope. Oscillometric blood pressure tests use a highly calibrated machine.
“If your smartwatch or smart ring consistently flags abnormal trends, that may be a sign to see a healthcare provider,” Dr. Morris said.
Wearables can track your sleep, including sleep stages, duration, disruptions, respiration and blood oxygen (SpO2). All these data points taken consistently over time can give clues to your healthcare provider about your heart health. Poor sleep is linked to high blood pressure, atrial fibrillation and increased cardiovascular disease risk. Sleep apnea is also a major and often undiagnosed risk factor for heart disease.
“Your smart device’s sleep monitoring does not take the place of a sleep study,” Dr. Morris said. “but it may help you improve sleep habits, like having a more consistent bedtime.”
Physical activity is one of the best things you can do for your heart. Even moderate consistent movement reduces cardiovascular risk. Adults need about 150 minutes a week of moderate physical exercise. You can break that up however you choose. Even “exercise snacks” have health benefits.
“Wearables can be a useful if they help you set wellness goals and work towards them,” Dr. Morris said. “it’s okay if you don’t hit your goals, but the key is to try to be consistent over time.”
You don’t have to be an Olympic athlete, but your activity level is one of the clearest signs of heart health.
One data point is nearly meaningless, but many data points, taken consistently over time, can build a pattern. If your wearable consistently gives you concerning readings, make an appointment, either with your primary care provider or a cardiologist. If you already have risk factors for heart disease, such as a family history, obesity, or a history of smoking, don’t wait for your wearable to sound the alarm.“The data and trends from your wearable can be a great tool to help you work toward improving overall health and wellness, and identify when an abnormal trends appears,” Dr. Morris said.
