How to Change Norton Healthcare Providers

Schedule an appointment online or call

It is not unusual for providers to move to a different location or retire. We want to ensure that changing doctors or other primary care providers is as seamless and convenient as possible.

Establishing Care With a New Primary Care Provider

There are a few ways you can become established with a new Norton Healthcare provider:

  • Option 1: Call your current practice to speak to our staff and schedule with another provider at your primary care location. Or call (502) 629-1234, option 3, to speak to someone who can help you find a new medical home at another Norton Community Medical Associates primary care location.
  • Option 2: If you have seen another provider at your current location before, log in to your Norton MyChart account to book an appointment with that provider.
  • Option 3: You can browse our primary care providers by your location and other characteristics and book online. Or start by reviewing locations across the Louisville area and Southern Indiana.

Arranging for Prescription Refills

For medication refills, you may call our location directly or request a prescription refill from your Norton MyChart account prior to your provider’s departure. If appropriate, our office will fulfill that request. If the prescription requires a provider visit, we will help schedule one for you.

Who Do I Say Is My Provider In the Meantime?

It’s not uncommon for specialists or other providers to ask the name of your primary care provider. You may list your Norton Community Medical Associates office until your new care is established.

Is There A Way to See My New Provider Sooner?

Yes, whenever you book an appointment with a primary care provider, you can opt in to our digital wait list through your Norton MyChart account.

Can I Still Come in Regularly for Routine Labs?

Yes, you may continue to visit our location for labs or receive drive-thru service at Norton Healthcare Express Services.

Will My New Provider Accept My Insurance?

All of our Norton Community Medical Associates primary care providers accept the same insurance plans, including Medicaid, Medicare and most commercial insurance plans.

What if I Need Urgent Care for an Illness?

Will My New Norton Healthcare Provider Have My Health Information?

Yes, your health information, including test results and more, is located securely within your Norton MyChart account. It stays accessible to you at all times as well as to Norton Community Medical Associates providers involved in your care. If you choose to seek care elsewhere, you can transfer your health information within MyChart.

It is our privilege to care for you, and we appreciate the trust you have placed in us. If you have any questions, reach out to your primary care location directly.
