Our certified educators are available to walk Norton Healthcare patients through your treatment plan, discuss lifestyle changes and connect you with our support group and community resources. Diabetes is complex, and having someone help you understand and navigate your care is one of the best ways to improve your health. Diabetes is not just what you do and don’t eat.

Let’s Chat is a virtual and in-person support group offered monthly through Zoom. Sessions are led by endocrinology provider Stacy L. Koch, APRN. Stacy, a nurse practitioner, provides education and an opportunity to connect and talk with other people who have diabetes. Each month there is a topic of focus — past topics have included coping with diabetes, healthy eating, exercise and medications.
Our pediatric and adult diabetes specialty providers work together to ease a young adult’s transition from Norton Children’s Endocrinology, affiliated with the UofL School of Medicine, to Norton Community Medical Associates – Endocrinology.
They work with patients to make this an easy and convenient process. As part of your transition, these providers will share medical records and care plans to help you feel confident and independent in your care. During your first transition visit, you will meet with your new provider, diabetes educator and nurse navigator. You will discuss treatment goals, concerns and medications.
