Medical Weight Loss and Bariatric Surgery Checklist

You may qualify for a weight loss procedure if you are:

  • Between ages 18 and 65
  • Body mass index (BMI) of 40 or above
  • Have tried or currently are trying to lose weight through other means, such as diet and exercise
  • Not drug or alcohol dependent
  • Psychologically ready for weight loss surgery and willing to commit to a new lifestyle
  • Struggling with a weight-related condition such as Type 2 diabetes, high blood pressure, heart disease or obstructive sleep apnea
