Carnet de vacunación de COVID-19

Acceda a su historial de vacunas COVID-19 en MyNortonChart

Los detalles del historial de vacunación de COVID-19 de un paciente – incluyendo las fechas de cada dosis de vacuna – están disponibles en MyNortonChart. Si ha perdido o extraviado su carnet de vacunación de COVID-19 y recibió su(s) vacuna(s) a través de Norton Healthcare, por favor utilice su cuenta de MyNortonChart para obtener su información de vacunación.

Siga estos pasos para acceder a su historial de vacunación:

  • Acceda a MyNortonChart a través de la aplicación MyChart o visite NortonHealthcare.org e inicie sesión
  • Haga clic en “Menú” (círculo con tres líneas apiladas). El icono de menú estará en la parte superior central de la aplicación y situado en la parte superior izquierda en línea.
  • Vaya a “Mi ”
  • Haga clic en “COVID-19.”
  • Haga clic en la línea verde COVID-19 para acceder a los mismos datos que figuran en la tarjeta de vacunación física que se le entregó en su cita de vacunación
    • Esto incluye las fechas de las dos dosis de su vacuna y el fabricante de la vacuna que recibió.

Si desea imprimir su información de vacunación, en este momento puede hacer una captura de pantalla e imprimirla.

Si está inmunodeprimido, puede recibir una dosis adicional (tercera) de la vacuna del mismo fabricante (Pfizer o Moderna) al menos 28 días después de la segunda dosis para personas de 18 años o más (a partir de los 12 años para Pfizer).

La Administración de Alimentos y Medicamentos (FDA) y los Centros para el Control y la Prevención de Enfermedades (CDC) están trabajando activamente para determinar pautas para las personas que recibieron la vacuna de Johnson & Johnson.
